The Intervention Plan (IP) and the Transition Plan (TP) play a major role in the organization of special education, health, and social services.
Although the PI and PT often refer to documents, they are first and foremost complex, multidimensional, and multidisciplinary educational or clinical processes that draw on a variety of cross-cutting and essential areas of knowledge.
The various individuals involved in the IP or PT process have numerous tasks, each of which is distinct from the others. However, they all share a common goal: to successfully develop, implement, and revise IEPs or TPs so that the learner or student can succeed academically, socially, and in terms of skills development.

Nathalie Myara, Ph.D. in educational psychology, is a lecturer and researcher in the Department of Educational Psychology and Andragogy, Faculty of Education, Université de Montréal. Her research focuses on the design and lifecycle efficiency of intervention plans (IP) and transition plans (TP), training in relation to the IP or TP lifecycle, dynamic assessment of special needs students, and intervention strategies, in particular cognitive mediation.
